More medical consultations are taking place by video or telephone instead of in person. Do the advantages of this development outweigh the disadvantages?

Video and telephone consultations can save travel and make routine follow-up easier, particularly for patients who live far from a clinic. In my view, these advantages outweigh the disadvantages only when remote appointments are used for suitable cases and patients can move promptly to an in-person physical examination when necessary.
Remote appointments can be valuable when a clinician has judged that the consultation is suitable for that format. Consider a patient who has been offered a routine follow-up by video and would otherwise need to travel a long distance. Avoiding the journey could make the appointment easier to fit around work or caring responsibilities. The patient would still have scheduled contact with a professional, but without the same transport arrangements or time away from home. This benefit concerns the practical effort of attending, rather than a promise of faster treatment or better results. It also depends on the patient being able to use the chosen format and having a suitable place for the conversation.
The main limitation is that remote contact cannot provide every part of an in-person appointment, including a direct physical examination. Clinical judgement must therefore guide whether that format is appropriate, rather than a booking system treating convenience as the deciding factor. Practical access is another concern: poor connectivity, limited privacy or difficulty using the platform can exclude some patients. A service could provide clear joining instructions and a way to report problems with the format. It would also need workable alternatives, arranged according to professional assessment and individual access needs. Offering a video link alone does not establish that the patient can use it or that the appointment meets the purpose for which it was arranged.
Overall, remote consultations can widen access and support timely routine follow-up, while their main risks arise when they are used beyond what the available evidence allows. With reliable triage, accessible alternatives and a direct route to physical examination, they are a useful addition to in-person care rather than an unsafe substitute for it.
